(HealthDay News) -- If dry, itchy scalp psoriasis is bothering you, there are things you can do to help bring it under control.

The American Academy of Dermatology advises:
  • Avoid scratching your scalp.
  • Treat the psoriasis to minimize the itching.
  • Be gentle when shampooing your hair; don't rub or scrub.
  • Gently remove scales from the scalp, taking care not to pick them off. Talk to your dermatologist about a skin softener.
  • Find healthy ways to manage stress.

Ixekizumab clears psoriasis in up to 80% of patients

Psoriasis is a chronic inflammatory disease caused by dysregulated immune responses. Recent advances in understanding the underlying cytokine networks has led to attempts to create drugs that can completely clear psoriasis plaques.
In three phase III trials, researchers explored the efficacy of ixekizumab, a monoclonal antibody against interleukin-17a, in a total of 3,866 patients with moderate to severe psoriasis.

They found that, after 12 weeks, 76.4–81.8% of patients who were randomly assigned to active treatment had minimal or clear psoriasis, compared with 3.2% of patients assigned to placebo. At week 60, 68.7–78.3% of patients who received ixekizumab had maintained their improvement.
In The New England Journal of Medicine (online, 8 June 2016), the researchers say the response rates are some of the highest ever seen from a psoriasis drug, but caution that long-term safety needs assessment.

Severe psoriasis can be completely cleared with a new drug: Studies

Psoriasis, a relatively common skin condition that causes patches of thick, red skin with silvery scales, can be found on various parts of the body. For many people it can be mild, but others experience severe psoriasis, which can cause a great deal of stress and self-confidence problems. Now, three large clinical trials seem to suggest a new drug can completely clear up severe psoriasis, offering hope to those who have been battling the skin condition.
Imagine for a moment what it would be like to live with pain, swelling, heat, and redness on your face, legs, lower back, palms, or the soles of your feet. This is where psoriasis can show up. For some people, the thick, red spots can even appear on genitals and inside the mouth. Now, consider after years of living with such a condition, there is hope thanks to an effective new treatment.
Three large, long-term clinical trials, led by Northwestern Medicine Investigator and professor of dermatology Kenneth Gordon, indicated that 80 percent of people with moderate to severe psoriasis experienced a complete or almost complete recovery from the skin condition with a new drug called ixekizumab. Gordon has reported that the results show “the great majority of responses persist at least 60 weeks”.
It is bad enough that psoriasis is itchy, uncomfortable, and unsightly, but experts say it is also linked to an increased risk of depression, heart disease, and diabetes. Finding effective treatment is important not only for comfort level, but overall good health.
According to those who worked on the clinical trials, the drug ixekizumab neutralizes a pathway in the immune system that promotes psoriasis.
To test the drug, clinicians enrolled close to 4,000 adult patients at more than 100 study sites in 21 countries. Everyone who took part had either moderate or severe psoriasis. Patients received either injections of ixekizumab at various doses or a placebo over a period of more than one year.
The team then assessed whether the ixekizumab reduced the severity of the skin condition compared to the placebo, and also monitored any adverse effects. By about 12 weeks, close to 82 percent of those with psoriasis had their skin condition updated to “minimal” or “clear”. By week 60, between 69 and 78 percent had maintained their improvement.
Just a few years ago, doctors thought complete clearance of this inflammatory disease was impossible. Now they see that based on the trials, they can anticipate 80 percent will have a good response to ixekizumab and about 40 percent will be completely cleared of psoriasis.
Since the results of the clinical trials were announced, the Food and Drug Administration (FDA) has approved ixekizumab.

NHS Choices said psoriasis affects around 2 per cent of people in the UK. It can start at any age, but most often develops in adults under 35 years old.
The condition occurs due to an over-reaction of the immune system, which causes inflammation and rapid growth of skin cells.
Skin cells are normally made and replaced every three to four weeks, but in psoriasis this process only lasts about three to seven days. The resulting build-up of skin cells is what creates the patches associated with psoriasis.

Experts have recommended five way to beat the painful skin condition.
Essential vitamins
Vitamin D, which people can get from the sun, is it vital for bone health as well as obesity and diabetes, but it can also help with psoriasis.
“Vitamin D can change the way cells grow,” said Shona Wilkinson, Nutritionist at SuperfoodUK.com.
“As those who suffer from Psoriasis tend to produce more skin cells, it can slow this process down and cause the plaques to become less scaly and thinner.
“It can be difficult to get enough sunlight to stimulate vitamin D synthesis in this country – even in the summer.
“The ideal first step is to get your vitamin D level tested to see how much yours needs to increase, if at all. In most cases, your doctor will do this for you. If you’re deficient (under 75 nmol/L) or ‘insufficient’ (under 100 nmol/L) then taking a supplement can be the best way to get the level up.”

Fish
Psoriasis is an autoimmune condition that causes inflammation and Omega 3 fatty acids, found in fish oil can be very helpful in reducing it - according to experts.

“Unfortunately, many people have much higher levels of omega 6 than omega 3 in their body, often due to the use of vegetable oils in cooking and the lack of oily fish consumption,” said  Dr Marilyn Glenville, nutritionist and author of Natural Alternatives of Sugar.
“To correct this balance, cut out all vegetable oils and margarines (as well as processed food) and replace it with healthy fats such as olive oil, coconut oil, butter or ghee.
“Aim to eat oily fish such as sardines, salmon or mackerel at least three times a week. For additional support, it may be helpful to take a good quality omega-3 fish oil supplement such as NHP’s Omega 3 Support.”

Keep the skin moist
To relieve skin from itchiness and discomfort, expert advise sufferers to keep the skin moist and nourished from the outside.
However, experts warn some emollient creams that are sometimes recommended by doctors and pharmacists, rarely provide long-term relief.
Cassandra Barns said: “They can even irritate already inflamed skin, as very often they contain chemicals, such as chlorocresol and cetearyl alcohol.

Psoriasis and smoking: Links and risks published by Dove Medical Press
